That's what the low-profile shape is for. The flanges sit inside the ear canal and the outer stem stays close to the ear, so there's very little sticking out for a pillow to press against. It's the main complaint people have about foam plugs, and the reason these are shaped the way they are.

Rinse in warm soapy water and leave to air dry. The silicone is non-porous, so nothing soaks in. Don't boil them.
